Regular and Extra Dividends
Form 3013 can be used whenever the board finds it appropriate to declare an extra dividend in addition to the regular dividend.~

Upon a duly made, seconded and unanimously carried motion, the following resolution was adopted by the Board of Directors of this Corporation:
WHEREAS, the financial statement submitted by the Treasurer of this Corporation to the Board of Directors reveals that the Corporation has a surplus of > dollars ($> ), for the fiscal year ending on the > day of > , 19>  , it is hereby
