I designate the above name Primary Beneficiary to receive the death benefits to which I am entitled under the above named Retirement Plan Trust. 2. Contingent Beneficiaries. In the event the Primary Beneficiary should predecease me, or in the event that our deaths should occur simultaneously, or in a common disaster or calamity, then, I designate the above named Contingent Beneficiaries to receive the amount to which I am entitled under the Trust. If more than one Contingent Beneficiary is named, they shall benefit equally, if no designation is given to the contrary. The Advisory Committee may require any affidavit or other evidence, in form satisfactory to the committee, from any Primary or Contingent Beneficiary as proof of such beneficiary's right to receive benefits from the Trust.
