Please first read the manual to learn all of the information
needed to complete the form, and in addition, the limitations
on the use of the form.

The following information generally is applicable to the 
document generator:


Fill in the blank boxes-- Enter the information. You can enter
over a single line worth of data. If you make an error before
entering the information you can use the backspace key to erase 
information. 

Multiple choice screens-- The choices are mutually exclusive, pick
the one which is applicable.

Yes/No screens-- Select yes or no.
